Fill out your Name, etc.
Check off the Merit Badges you already have so they don't assign you to a MB you already have earned.
Rate your choice of selections from 1 to 7 (there is a 90% chance you will get your top 3)

Bring your $20 payment to the next Troop Meeting Dec, 14.
